I have no idea why you would even want to get your useless biped form back in the first place, but if you somewhat decide to reject this gift from the gods, they won't let you get your old self back if you don't restart the game beforehand.
(after disabling the mod, your character may become invisible while walking, you have to restart the game to fix that)
