Getting hypothermia in vanilla will slowly start to drain your health - it can drop to very low levels but won't kill you. I give you two options to change that :

  • Main Version - gives that final touch at the end to kill you. It is randomised so it may take 10 minutes to several hours(in-game time). It only becomes dangerous when you already have the health drained previously by the vanilla hypothermia system. This is still quite hard to accomplish in my opinion. You'd really have to be in the cold for long time without any chances of fire or getting inside somewhere.
  • Hard Version - starts to randomly drain health every 10 in-game minutes if you have the Freezing/Hypothermia moodlet AND ONLY if your general health is already below 15. This ensures coming out of the cold won't be as quick, as you have to get past the freezing phase as well. If you get your temperature up to 'Cold' then you are clear and even if below 15 health you won't get any extra health drain. It also adds another dimension to it if you were to have somewhat very poor health already. I personally suggest this version.

IMPORTANT: The preview video is for show only - to let you know it can be done. The actual process can't be demonstrated with a short clip as the vanilla ColdDamageStage mechanic ticks in real time and fast forwarding in Debug would give wrong results. Time scale is similar tho.

This is part of... :

Project Realistic Overhaul - Set of mods I've created that will overhaul the vanilla game experience to be more hardcore and true to life, but on the other hand leaves you the option to make it through and survive. It makes items and actions have more value and makes the world more alive.
